چکیده

Patients who reattend the accident and emergency (A&E) department after a minor head with persistent symptoms of headache, dizziness, and fatigability are diagnosed commonly as suVering from postconcussional syndrome. Clinicians should be aware, however, that minor head injury may unmask previously asymptomatic neurological diseases, including tumours, berry aneurysms, arteriovenous malformations, and cerebral abscesses. We report such a case of asymptomatic cerebellar medulloblastoma presenting after minor head injury. A 17 year old man presented five days after receiving a forceful blow to the head from a football, which resulted in him feeling dazed momentarily. He had experienced no neurological symptoms before this event but described a history of headache and dizziness persisting from the time of injury. Initial examination revealed a Glasgow coma score of 15. Full neurological examination revealed no abnormality and he was diagnosed as having postconcussional syndrome and discharged with head injury advice. He returned four days later with symptoms of increasing headache, vomiting, dizziness, and drowsiness. He was admitted to the A&E short stay ward where further examination revealed a Glasgow coma score of 15. Repeat neurological examination revealed bilateral papilloedema and up-beat nystagmus. Computed tomography of the brain with contrast (fig 1) showed a hyperattenuating mass in the right cerebellar hemisphere with distortion of the fourth ventricle, and triventricular hydrocephalus. He was transferred to the regional neurosurgical centre and underwent surgical excision of a large multilobular tumour from the superior aspect of the cerebellar vermis. There were no complications and his hydrocephalus settled rapidly. Subsequent histology showed the mass to be a cerebellar medulloblastoma or primitive neuroectodermal tumour. The mechanism whereby asymptomatic brain tumours are unmasked by minor trauma is unclear. It is feasible that direct trauma to the tumour may result in oedema and hydrocephalus leading to symptoms resembling the postconcussional syndrome. Although cerebellar medulloblastoma is rare in adults, postconcussional syndrome is common. However patients who reattend after minor head trauma have been shown to represent a high risk group. Accordingly a short period of observation should always be considered and early computed tomography undertaken in those whose symptoms persist.

Cerebellar Infarction in Childhood: Delayed-Onset Complication of Mild Head Trauma

Objective Cerebellar ischemic infarction is a rare complication of minor head trauma. Vertebral artery dissection, vasospasm or systemic hypo perfusion can cause infarct. However, underlying causes of the ischemic infarct cannot be explained in nearly half of cases. The accurate diagnosis is essential to ensure appropriate treatment. Here we report a five yr old boy patient of cerebellar infrac...

Post concussion ataxia following minor head injury.

A 2-year-old girl presented with history of fall from a 2 feet high bed, 20 days back. There was no alteration of consciousness immediately but four hours later parents noticed unsteadiness of gait and shaking of head and neck. These progressed and she became bedridden due to ataxia 7 days later. There was no pain or restriction of neck movement. There was no preceding viral illness, vaccinatio...

Bilateral retrocerebellar arachnoid cysts exerting mass effect and associated with cerebellar tonsillar ectopia in an otherwise healthy adult.

Rarely, midline or unilateral posterior fossa arachnoid cysts (ACs) exert local mass effect resulting in the symptoms and signs of cerebellar and brainstem dysfunction. These cysts are sometimes seen in conjunction with cerebellar tonsillar ectopia (TE), although the relationship between these two entities is unclear. Bilateral ACs in the posterior fossa are virtually unprecedented. We describe...

Continuous Theta Burst Stimulation (cTBS) on Left Cerebellar Hemisphere Affects Mental Rotation Tasks during Music Listening

Converging evidence suggests an association between spatial and music domains. A cerebellar role in music-related information processing as well as in spatial-temporal tasks has been documented. Here, we investigated the cerebellar role in the association between spatial and musical domains, by testing performances in embodied (EMR) or abstract (AMR) mental rotation tasks of subjects listening ...

Posttraumatic Cerebellar Infarction after Repeated Sport-related Minor Head Injuries in a Young Adult: A Case Report

A healthy 23-year-old man suffered helmet-to-helmet collisions with an opponent during American football game twice within 3 days. He then experienced continuous vomiting and dizziness. Magnetic resonance imaging revealed acute infarction in the right cerebellar hemisphere, and magnetic resonance angiography revealed transient stenosis of the right superior cerebellar artery. Although minor hea...
